What to include

Transaction hashes for every transfer related to the loss — across all chains. Platform name(s) involved, including any social media or messaging accounts where contact was made. Approximate dates of the first contact, the loss event, and any post-loss communications. Loss range in USD-equivalent at time of loss. Any KYC documents you provided to the platform, even partially.
